A note about the Forms
The forms have been provided in a number of formats:
1. Word Document - these forms should be printed and completed by hand.
2. PDF Form - This is an image of the document. The document can be viewed on the desktop and printed using Adobe Acrobat reader software.
3. Word Form - This is a Microsoft Word Form document. The document can be viewed and completed on the desktop using Microsoft Word software, version 2000. Download and save the document as a template on your computer system for completion from the desktop.
Forms can be on-forwarded to clients attached to an E-mail. To on-forward the form, display the required form on your desktop. From the Toolbar, click on:
File - from the drop-down table, select;
Send - from the drop-down table, select;
Page by E-mail.
This will open your Contacts list to select the contacts or to enter the E-mail address of the recipient.
Prepare your E-mail in the unsual manner. When completed, click on "Send".
